EU-US & SWISS PRIVACY SHIELD

Seven Corners, Inc. complies with the EU-U.S. Privacy Shield Framework and the Swiss-U.S. Privacy Shield Framework(s) i.e. Privacy Shield as set forth by the U.S. Department of Commerce regarding the collection, use, and retention of personal information transferred from the European Union and the United Kingdom and/or Switzerland, as applicable to the United States in reliance on Privacy Shield. With regard to  your relationship with us (e.g. consumer insured or policyholder; producer or appointed representative; or other person relating to our business), we may collect the following personal information from you and your dependents:

  • General Information and Contact information

Name; contact information such as email, mailing, or alternate address; telephone number; mobile number; fax number; telephone recordings; gender; photos; marital status; family status; date of birth; physical attributes; activity records such as exercise routines or driving records; relationship to the policyholder, insured, or claimant; beneficiary information; employment history; education; qualifications and experience; professional licenses.

  • Identification numbers issued by government bodies or agencies

Social Security or national insurance number; passport number and citizenship information; military identification number; or driver’s or other license number.

  • Financial information

Payment card number and billing address; bank account or other financial account number and account details; credit history and credit score; assets; income; and other financial information.

  • Medical Condition and health status

Your medical practitioner’s name, address, and contact information; current or former medical condition and health status including other factors affecting insurability; date and cause of death, injury, or disability; medical procedures performed; personal habits (smoking or consumption of alcohol); and prescription information.

CHILDREN’S PRIVACY

The Seven Corners family of websites is not targeted to and does not intentionally collect personal information from children under the age of 18. HOW WE SECURE PERSONAL INFORMATION

  • We utilize industry standard security safeguards, including maintaining physical, electronic, and procedural safeguards, in compliance with applicable laws to protect personal information from loss and unauthorized access, modification, disclosure, inappropriate alteration or misuse.
  • Employees are trained on the importance of privacy including prohibiting the disclosure of any customer information to unauthorized individuals or parties.
  • Electronic customer files are kept in a highly secure environment with restricted access within a secure building. Paper-based files are stored in a secure building and are further restricted by locking filing cabinets. Access is restricted to authorized employees who have a legitimate business purpose and reason for accessing your personal information.
  • To guard against unauthorized access, you are required to sign-on to your accounts and secured areas of the Seven Corners websites using a User ID and a password.  Both the User ID and password are encrypted when sent over the internet.  You are responsible for protecting your own information.  Do not share your User ID or password with anyone. If you suspect that someone is using your User ID or password, please report it immediately by sending an e-mail to privacy@SevenCorners.com. Always log out of our website to keep your information and account secure.  Protect your computer and environment with virus protection and other safeguards to make sure others cannot access your information through your computer.  Seven Corners will not be liable for disclosures of your personal information due to errors in transmission or unauthorized acts of third parties.
  • When you call us for patient and specific product questions, you will be required to verify your identity by providing personal information. We will only use this information to identify you.
  • While we take reasonable steps to protect personal information transmitted electronically as outlined in this notice, we cannot guarantee our ability to do so as no internet transmission is 100% secure.
  • We securely destroy, erase or de-identify personal information when it is no longer required to fulfill our services and commitments to you or our contractual obligations following applicable business, legal, or regulatory requirements.

  • You can add, correct, or update certain personal information.  Please note that before we can provide you with any information or correct any inaccuracies, we may ask you to verify your identity and to provide other details to help us respond to your request. We will only use this information to verify your identity. Where appropriate, we will communicate these changes to other parties who may have received incorrect information from us.

Additionally, certain business partners may require that you contact them to correct your personal information; in this case we will notify you and provide the appropriate contact information.

  • Where allowed by law, you may request access to personal information and erasures of personal information we store about you.  There may be circumstances when we are unable to provide access to your personal information. We may deny, or restrict access for legally permissible reasons, such as situations when the information contains references to other individuals and is not reasonably severable, or when it cannot be disclosed for legal, security, or commercial proprietary reasons.  We will advise you of any reason for denying or restricting an access request. COURT ORDER PROTECTION REQUESTS

To submit a valid court order of protection and/or a request to receive communications of claim-related information by alternative means or at alternative locations to Seven Corners, Inc., please send the following information to Seven Corners, Inc.

1.        Policy information if available;

2.       Any alternative address, telephone number or other method of contact; and

3.       A valid court order of protection issued by a State Court and/or a statement that disclosure of claim-related information could endanger the person seeking confidentiality.
