The ASPE is a self -funded, limited health care benefit plan designed to pay covered medical expenses for eligible Exchange Participants. Covered medical expenses are subject to limitation, and pre-existing conditions are not covered. This plan is NOT an insurance policy.

IMPORTANT:
In the US, when you call a doctor’s office for an appointment or present your ID card to a provider, it is important for you to say: “My healthcare coverage utilizes the First Health International PPO Network. Are you a First Health International provider?”

DO NOT SAY:
“I have Seven Corners’ Insurance” or “I use Seven Corners’ Network” or “I have ASPE insurance.” Providers WILL NOT recognize this language and your coverage might not be accepted.

ASPE requires all of its members (exchange participants) to pay following co-pay for all office visits, ER, urgent care and hospitalizations. The provider will require the co-pay at the time of visit. As a reminder the co-pay amount will be pre-printed on your ASPE ID card.

  • $25.00 co-pay for office visits
  • $75.00 co-pay for ER, hospitalizations and urgent care
  • $15.00 co-pay for all brand name drugs with a generic equivalent available.

Pre-certification required

  • This healthcare plan requires pre-certification for all inpatient hospital admissions, skilled nursing, outpatient chemo and radiation therapy, outpatient surgeries and procedures, pregnancies, physical or occupational therapy, dialysis, plasmaphoresis, MRI, PET scan, CT, home health and home infusion therapy.
  • You or your physician must call the pre-certification phone number, 1.800.461.0430, to obtain pre-admission approval at least one business day before a planned hospitalization. Certification for emergency admissions or admissions due to an unexpected illness or injury must be obtained within two business days following admission. Pre-certification is not a guarantee of coverage. A $300 penalty will be applied if pre-certification is not obtained.

  • You need to confirm coverage and benefits
  • As soon as non-emergency hospitalization is recommended
  • Within 48 hours of the first working day following an emergency admission
  • Your physician recommends any surgery including outpatient
  • Prior to any treatment for dental pain
